Policies and Payment
Freitag Family Chiropractic does not participate with private insurance
- It is our policy at Freitag Family Chiropractic to collect payment at the time of service from every patient.
- Private Health Insurance: Freitag Family Chiropractic is Out-Of-Network for all Health Insurance Plans. If requested, we will provide a superbill for you to submit claims directly to your insurance company for reimbursement if your policy covers out-of-network services.
- Medicare: Dr. Freitag is NOT a Medicare Provider. Medicare will not reimburse you for services provided at Freitag Family Chiropractic.
- Workers Compensation: Freitag Family Chiropractic does NOT accept Workers Compensation cases, but we’d be happy to refer you to a chiropractor that does, if needed.
- Auto Accident: Freitag Family Chiropractic accepts Auto Accident / Personal Injury cases, however, patients MUST provide payment at the time of service. We will provide a superbill for you to submit to your auto/liability insurance carrier for reimbursement.
- HSA & FSA Payment: Freitag Family Chiropractic accepts HSA and FSA forms of payment.
- Electronic Payment: Freitag Family Chiropractic accepts electronic and contactless forms of payment.
These policies allow us to:
- Spend more time with you during your visit.
- Treat the areas of your body that need addressing that day, not just your spine or your initial area of complaint.
- Provide preventive, maintenance or chronic injury care, without restrictions.
- Provide affordable care plans for wellness care.
- Minimize contact during transactions.
New Patient Paperwork
Download, print, and complete Initial Intake Paperwork and any of the Questionnaires related to the pain you have.
First Visit Expectations
If you have not already printed out and completed the Initial Intake Paperwork above, please arrive at our office 15 minutes before your scheduled appointment time.
During your Initial Visit Dr. Freitag will:
1) Review your completed paperwork with you to gather a comprehensive view of your main complaint(s) and health history.
2) Perform a focused physical exam to identify the likely cause of your pain.
3) Outline your treatment options and provide recommendations on care.
4) If appropriate, will perform a chiropractic treatment or "adjustment"
5) Dr. Freitag will provide a referral to other specialists as needed.
Please bring any recent x-ray, MRI or CT scan images and reports and lab test results, related to the problem for which you are being seen.
Please wear comfortable clothing (sweat/athletic pants or shorts and socks) to each visit.
